This 8 3/4" x 4" comes with a glossy full-color cover sheet and 12 two-sided month-on-a-page calendar sheets. Easel back is white cover stock with practice logo and information printed in full color on two sides. Pre-scored back folds into an easel to set on a desk. Submit 12 photos to have photos on each sheet.
Flat size for mailing: 8 3/4 x 5 3/4
